We provide professional care and comfort
to our patients from conception to natural death.
On 1/1/2021, I shall be retired, officially. Back in October 2020,
I sent out letters to hundreds of my patients informing them
to seek out a new primary care provider. I shall no longer see
private practice. I shall stop refilling medications.
If you need medical records sent anywhere, the first CD-ROM
is free, and any subsequent one will be charged $5.00 per disc.
You need to call the main office number, 920-347-1990, and leave
a message on the answering machine: (1) Your full name, (2) the
destination for the disc with full address, and phone number if
available. I send files in pdf format, which is open source from
Adobe, and can be read by any DVD reader and any standard
computer. You may output any of the files on paper. There are no
executable files on the disc.
